Milan, 15 Mar. (Adnkronos Salute) — «World first» cardiac surgery in Padua, where a 62-year-old man with severe mitral regurgitation and atrial fibrillation was subjected simultaneously to valve plastic and fibrillation ablation with auricle closure, all with a beating heart without extracorporeal circulation. «Rapid and immediate» the recovery of the patient, at his first heart surgery — report from the University Hospital of Padua — discharged «without complications after a few days» from the operation lasted about 4 hours and performed a few weeks ago by the director of Cardiac Surgery, Gino Gerosa, and his team.
The intervention was studied and planned also creating a transparent resin model with the same characteristics as the patient’s heart, explains a message. The cardiac surgeons operated by performing a minithoracotomy of about 5 centimeters in the fifth left intercostal space. Once they entered from the tip of the heart, they corrected mitral regurgitation by applying some artificial neochords in goretex. Then, under thoracoscopic guidance, the left auricle was closed with the Lariat system, a kind of lace. Subsequently, always with the vision of the thoracoscope that magnifies the field, epicardial isolation of the pulmonary veins was performed, that is, the ablation of the posterior wall of the left atrium. The patient’s normal sinus rhythm was thus restored.
Together with Gerosa the cardiac surgeons Vincenzio Tarzia, Augusto D’Onofrio and Luigi Garufi, he lists the Aou. A great contribution — he highlights — was provided by the cardiac anesthesiologist Demetrio Pittarello who, through 3D three-dimensional transesophageal echocardiography, provided a perfect view of the mitral valve and cardiac cavities. Among specialized technicians of extracorporeal perfusion, operating room nursing staff and social health workers, a dozen professionals collaborated with the surgical team.
«The absolute novelty of the intervention carried out, of very high complexity — reads the note — is given by the double simultaneous intervention for two important problems resolved through the arrangement and restoration of the functionality of the mitral valve, the concomitant correction of atrial fibrillation with closure of the auricle always carried out in the same intervention, without ever stopping the heart and without the use of extracorporeal circulation «.
The operation is for the Aou a «clear example of microinvasive cardiac surgery carried out by the Cardiac Surgery of the University Hospital of Padua, a high-volume center of interventions with a personalized approach to the patient, which is not new in devising cutting-edge procedures, pioneers that open new paths to highly complex surgical interventions».
